By subscribing, you are entering into a 12 month membership contract with Totality Medispa. I authorize Totality Medispa to charge my credit card $39/month for 12 months.

Club Evolus is a monthly membership for patients who want regular tox treatments at a predictable cost. $49/month, no enrollment fee.

How it works:

• 20 units of Jeuveau per session, with a minimum of 90 days between treatments
• Eligible for your first treatment after 3 monthly payments — or prepay to get in sooner
• Units do not roll over: waiting longer than 90 days does not accumulate extra units

Important rules:

• Cannot be combined with other Evolus offers
• Enrolling in Club Evolus opts you out of the separate Evolus Rewards program
• Membership is non-transferable

Cancellation:

• You can cancel anytime; cancellation takes effect immediately
• $100 cancellation fee if cancelled within the first 3 months
• All payments are non-refundable
• You may reactivate your membership after cancellation

Best fit for patients who come in every 3–4 months for consistent neurotoxin maintenance.

After 3 months of membership is paid:
